Please download and complete the new patient paperwork (link below) before your first appointment (requires Adobe Reader). These can be filled out electronically and returned via email, or printed out and brought to your appointment. If you need assistance or cannot complete them in advance, please arrive 20 minutes before your scheduled appointment time to fill them out here.
New Patient Forms
We also request you bring the following to your first appointment:
- Photo ID and insurance card(s)
- Referral from healthcare provider (if provided; not required for evaluation)
- Written reports from diagnostic tests (CT scans, MRI, VNG, etc.)
- Medication list*
*Medication list should include dosage and frequency of prescribed and over-the-counter medications, as well as vitamins and supplements.
We are contracted Medicare providers and can bill out-of-network with most PPO plans. We also accept Worker’s Compensation and private pay.
As a courtesy, we will verify your insurance coverage prior to your first appointment and bill your insurance company for treatment.
Deductibles, co-insurance, and payment for products or services not covered by insurance must be made at the time of your visit.

For your convenience, we have provided our New Patient paperwork in a PDF form. Please download and complete these forms in preparation for your first appointment. We will collect them when we check you in for your evaluation.
Cancellations and No Shows
Our office requires 24 hours notice to cancel or reschedule an appointment. As with other medical practices you visit, we need adequate notice if you aren't coming to a scheduled appointment. This allows us to offer the appointment time to another patient and maintain the therapist's schedule.
Late cancellations and no-shows are subject to a $50 fee.
