Vortex Physical Therapy & Balance
At Vortex Physical Therapy & Balance, we are dedicated to your recovery. We treat vertigo cases that resolve in 1-2 visits to severe vestibular and balance impairments that require months of therapy; no two situations are alike. In order to make every experience a positive one, we have detailed information about what you can expect during treatment here.
Preparing for your first visit
If you have already completed your New Patient paperwork, please arrive 5 minutes early for registration. However, if you need to complete paperwork, you should arrive 20-30 minutes early.
You will also need to bring:
1. referral / Rx from your doctor
2. Driver’s license or state ID
3. Insurance cards
4. current list of medications with dosage and frequency (including vitamins/supplements).
How to Dress
Vortex does not have a specific dress code for physical therapy. We suggest dressing comfortably in clothes that allow you to move. Women should avoid wearing skirts or dresses. Footwear should be comfortable and appropriate for walking. Please do not wear heels, open-backed sandals or slides, or slipper shoes.
Vortex is contracted with Medicare and we can bill most PPO plans with out-of-network coverage. We also accept Worker’s Compensation and private pay.
As a courtesy, we will verify your insurance coverage prior to your first appointment and bill your insurance for treatment.
Deductibles, co-insurance, and payment for products or services not covered by insurance must be made at the time of your visit.
For your convenience, our New Patient paperwork is available to download as a PDF form. Please download and complete these forms in preparation for your first appointment. You can email them ahead of time or bring them to your evaluation.
Cancellations & No Shows
We require 24 hours notice to cancel or reschedule an appointment. As a small private medical practice that reserves a full hour for your care, it is critical you provide adequate notice if you cannot come to a scheduled appointment. Otherwise, it leaves a hole in your therapist's schedule that cannot be filled.
Late cancellations and no-shows are subject to a $50 fee, which you will be expected to pay at the next appointment.
