Vortex Physical Therapy & Balance

At Vortex Physical Therapy & Balance, we are dedicated to your recovery. We treat vertigo cases that resolve in 1-2 visits to severe vestibular and balance impairments that require months of therapy; no two situations are alike. In order to make every experience a positive one, we have detailed information about what you can expect during treatment here.

Preparing for your first visit

If you have already completed your New Patient paperwork, please arrive 5 minutes early for registration. However, if you need to complete paperwork, you should arrive 15-20 minutes early.

You will also need to bring:
1. referral / Rx from your doctor
2. Photo ID (example: driver’s license, passport)
3. Insurance cards
4. current list of medications (and vitamins) with dosage and frequency.

How to Dress

There is no dress code, but we advise you to dress comfortably with safe walking shoes. Ladies should avoid wearing a skirt or dress.

Insurance Coverage

We are contracted Medicare providers and can bill out-of-network with most PPO plans. We also accept Worker’s Compensation and private pay.

As a courtesy, we will verify your insurance coverage prior to your first appointment and bill your insurance company for treatment.

Deductibles, co-insurance, and payment for products or services not covered by insurance must be made at the time of your visit.

Cancellations and No Shows

Our office requires 24 hours notice to cancel or reschedule an appointment. As with other medical practices you visit, we need adequate notice if you aren't coming to a scheduled appointment. This allows us to offer the appointment time to another patient and maintain the therapist's schedule.

Late cancellations and no-shows are subject to a $50 fee.
